Tiivistelmä
Rintasyöpä on yleisin todettu syöpä naisilla ja siihen sairastuu vuosittain noin 5000 naista Suomessa. Heistä noin 135 on alle 40-vuotiaita rintasyövän löytyessä. Maailmanlaajuisesti rintasyöpä tunnetaan yleisimpänä pahalaatuisena syöpänä, johon joka 20. nainen sairastuu elämänsä aikana. Rintasyövän esiintymismuotoja on useita erilaisia, joten diagnoosit ja hoidot ovat yksilöllisiä.
Imetys on luonnollisin tapa ruokkia vauvaa ja äidin rintamaito onkin parasta mahdollista ravintoa vastasyntyneelle. Sairastetun rintasyövän jälkeen imetykseen liittyy niin fyysisiä kuin sosiaalisiakin haasteita, kuten epävarmuus ja huoli maidon riittävyydestä sekä syövän uusiutumisesta. Fyysisiä haasteita imetyksen onnistumiselle voivat olla muutokset rinnoissa säde- tai leikkaushoidon tai koko rinnanpoiston vuoksi.
Opinnäytetyö toteutettiin narratiivisena kirjallisuuskatsauksena. Kirjallisuuskatsauksen tutkimusaineisto haettiin terveysalan yleisimmistä tietokannoista PubMed, Cinahl Ultimate sekä Medic. Tutkimusaineistoksi valittiin yhteensä 11 tutkimusta käyttäen määrittämiämme sisäänotto- ja poissulkukriteereitä sekä manuaalista rajausta hyödyntäen Hawkerin arviointityökalua. Tieteellisiä artikkeleita analysoitiin ja niistä pyrittiin löytämään yhteneväisiä tuloksia.
Opinnäytetyössä selvitettiin rintasyövän ja imetyksen yhteyttä toisiinsa, etenkin jo sairastetun rintasyövän mahdollista vaikutusta imetyksen onnistumiseen. Aiheesta on saatavilla hyvin vähän tietoa, joten kirjallisuuskatsauksen valmistumisen myötä tuotettu tieto koettiin merkittäväksi saada muun muassa hoitohenkilökunnan käyttöön. Helposti saatavilla olevan tiedon ansiosta hoitohenkilökunta pystyy helpommin ottamaan puheeksi imettämisen rintasyöpäpotilaan kanssa. Tämä mahdollistaa myös laadukkaamman potilasohjauksen ja -neuvonnan.
Tutkimustulokset osoittivat, että tietyt syöpähoidot estävät imetyksen turvallisuussyistä. Hoitomuodoista kemoterapia ja hormonaaliset hoidot vaikuttavat koko äidin elimistöön niin, että imetys on vasta-aiheista hoitojen ollessa käynnissä. Syöpähoidon ollessa paikallista vain toiseen rinnoista voi imetys onnistua yhdellä terveellä rinnalla.
Toimeksiantaja opinnäytetyössä oli Rintasyöpäyhdistys, Europa Donna Finland ry, joka pystyy valmiin opinnäytetyön pohjalta ohjaamaan rintasyöpäpotilaita ja tuottamaan aiheesta materiaalia verkkosivuilleen. Breast cancer is the most commonly diagnosed cancer in women, and approximately 5000 women in Finland fall ill with it each year. Approximately 135 of them are under 40 years old when breast cancer is found. Worldwide breast cancer is known as the most common malignant cancer, with one out of 20 women falling ill with it during her lifetime. There are several different forms of breast cancer, so diagnoses and treatments are individual.
Breastfeeding is the most natural way to feed a baby, and mother's breast milk is the best possible food for a newborn. After breast cancer, breastfeeding is associated with both physical and social challenges, such as uncertainty and worry about the sufficiency of milk and the recurrence of the cancer. Physical challenges to breastfeeding success can be changes in the breasts due to radiotherapy or surgery or a total mastectomy.
The thesis was implemented as a narrative literature review. The literature review´s research material was retrieved from the most common databases in the health sector, which were PubMed, Cinahl Ultimate and Medic. A total of 11 studies were selected as research material using our defined inclusion and exclusion criteria and manual cropping utilizing Hawker's assessment tool. Scientific articles were analyzed trying to find consistent results.
In the thesis, the connection between breast cancer and breastfeeding was investigated, especially the possible effect of already diagnosed breast cancer on the success of breastfeeding. There is very little information available on the subject, so the information produced by completing the literature review felt to be important to be made available to nursing staff, for example. Due to the easily available information, the nursing staff could bring up breastfeeding with the breast cancer patient more easily. This also enables better quality patient guidance and counseling.
The results showed that certain cancer treatments prevent breastfeeding for safety reasons. Among the treatments, chemotherapy and hormonal treatments affect the entire mother's body so that breastfeeding is contraindicated while the treatments are in progress. When the cancer treatment is localized to only one of the breasts, breastfeeding can be successful with one healthy breast.
The client organization for the thesis was the Breast Cancer Association, Europa Donna Finland ry, which, based on the completed thesis, is able to guide breast cancer patients and produce material on the subject for its website.
