Tiivistelmä
Suomessa päihteitä käyttäville suunnattu vertaistuki pohjautuu pitkälti 12 askeleen vertaistukiohjelmiin, joita ovat mm. AA (Anonyymit Alkoholistit) - ja NA (Anonyymit Narkomaanit)-ryhmät. Nämä ryhmät eivät kuitenkaan tavoita kaikkia vertaistukea tarvitsevia henkilöitä. Esimerkiksi henkilökohtaiset tavoitteet voivat olla ryhmien periaatteiden vastaisia, jonka vuoksi ryhmien ulkopuolelle jäädään myös vapaaehtoisesti, vaikka avuntarvetta koetaan. Tällaisia tavoitteita voivat olla mm. huumeiden käytöstä koituvien haittojen vähentäminen tai käyttömäärien vähentäminen lopettamisen sijaan.
Opinnäytetyön tavoitteena oli kuvata, millaisia stigmatisoivia elementtejä 12 askeleen perusteokset sisältävät. Stigmaa sisältäneet kohdat, eli löydökset sijoitettiin ylä- ja alaluokkiin. Löydöksien julki tuomisella pyrittiin tuomaan näkyväksi syitä, miksi ihmiset saattavat jättäytyä vertaistuen ulkopuolelle. Löydöksiä tuotiin esiin myös kaavioiden kautta.
Tutkimuksen aineisto kerättiin kahdesta 12 askeleen perusteoksesta, NA:n Se toimii – Miten ja miksi sekä AA:n 12 Askelta ja 12 Perinnettä. Aineiston analyysi toteutettiin kirjallisuuden pohjalta tehtynä sisällönanalyysinä sekä kvantitatiivisesti, että kvalitatiivisesti. Diskurssien tutkimisessa tukeuduttiin diskurssi- ja kategoria-analyysin periaatteisiin. 12 askeleen vertaistuen ulkopuolelle jääneeltä henkilöltä kerättiin kommentti työn tuloksista.
Stigmatisoivia kohtia löydettiin hyvin paljon ja kirjojen välillä oli myös stigman osalta painotuspoikkeamaa, vaikka molemmat kirjat toimivat näennäisesti samojen periaatteiden mukaan. Vertaistuen ulkopuolelta olevan kokemusasiantuntijan kommentti vahvisti tehtyjen havaintojen merkitystä ja toi toisaalta myös kokemustietoa tulosten tulkintaan. Muita vertaistuen muotoja suunniteltaessa stigma on tärkeää ottaa huomioon, jotta vertaistuki voi tarjota mahdollisimman monelle mahdollisuuden osallisuuteen. In Finland, the peer support for people who use drugs is mainly based on 12-step programs like AA and NA. Unfortunately, these groups cannot reach everyone who needs help. Personal goals, for example, can be against the group’s principles, which is why some people decide to keep themselves out of the groups voluntarily, even though they might feel the need of help. The personal goals can relate to minimizing but not completely stopping drug use or they can relate to harm reduction, for example.
The purpose was to portray what kind of elements of stigmatization the 12-step program’s books included. The findings were then put into upper and lower categories. The goal was to make the reasons why people might choose to stay out of these programs visible. Charts were used to make the findings more apparent.
The study material was gathered from the Finnish versions of NA’s and AA’s books which narrate their discourses on 12-step program. The analysis of the material was conducted by methods of content analysis which was used on quantitative and qualitative ways. Discourse analysis and category analysis principles were applied on the content analysis of the discourses presented in the books. A comment from a person outside of the 12-step programs was collected as part of the study.
Stigma was presented throughout both books and the books had some differences on what type of stigma was underlined the most, even though both NA and AA act under the terms of 12-step programs. The comment from an expert of experience verified the findings and it also brought some experienced knowledge into the interpretation of the results. It is important to acknowledge the existence of stigma if new kinds of peer support are piloted or planned. That ensures that the services reach as many people as they can.
