Tiivistelmä
Childbirth injuries include perineal tears and an incision made in the perineum, i.e. the episiotomy. Almost any woman having a vaginal birth can tear. During childbirth, most tears occur in the vagina and the perineum, but they can also extend into the urethral opening and the anal sphincter. The tears are due to intense stretching of the tissue during childbirth. The risk of tearing increases in the case of a large size or a malpresentation of the foetus, a vacuum extraction delivery and a prolonged second stage of labour. Tears are classified into four degrees according to their severity. A first-degree tear involves the mucous membrane, and a second-degree tear affects the pelvic floor muscles. A third-degree tear occurs in the anal sphincter and a fourthdegree tear extends into the rectal mucosa. The third and fourth-degree tears are less common.
The term episiotomy stands for an incision of the perineum in which the vaginal opening is enlarged in order to avoid severe tearing. An episiotomy can be performed in the case of e.g. a large size or a malpresentation of the foetus, a vacuum extraction delivery and if there is a significant amount of scar tissue in the perineum.
Childbirth injuries can be sutured in multiple ways. Suturing is a sterile procedure and is usually performed with absorbable sutures. Interrupted and continuous sutures are the most used techniques to suture childbirth injuries. In addition, a mattress suture technique and sutures in the subcutaneous tissue can be used.
This thesis was functionally executed. The purpose of this thesis is to improve the knowledge of midwives of the suturing techniques used to treat childbirth injuries. It also aims to support independent practice of suturing techniques and knot-making by producing an instructional video on the subject. This thesis is aimed at increasing the skills of suturing techniques and improving the self and classroom study of midwifery students. Better suturing skills contribute to the confidence and proficiency of midwives in treating childbirth injuries and restoring normal anatomy of women after childbirth. Synnytysvauriot käsittävät perineaaliset eli välilihan alueen repeämät sekä välilihan leikkauksen eli episiotomian. Lähes jokainen alateitse synnyttävä äiti voi saada repeämän. Synnytyksessä repeämiä voi tulla emättimeen, välilihalle, virtsaputken suulle tai peräaukon sulkijalihakselle. Tavallisesti repeämät syntyvät emättimeen ja välilihan alueelle. Repeämät syntyvät kudoksen joutuessa kovalle venytykselle synnytyksen aikana. Repeämille altistavia tekijöitä ovat sikiön tarjontavirhe, suuri koko, imukuppisynnytys sekä pitkittynyt ponnistusvaihe. Repeämät jaetaan neljään asteeseen. Ensimmäisessä asteessa vaurio kohdistuu limakalvolle ja toisessa asteessa vaurio syntyy lantionpohjan lihaksiin. Kolmannen asteen repeämässä vaurio on syntynyt peräaukonsulkijalihakselle ja neljännessä asteessa peräsuolen limakalvoon asti. Kolmannen ja neljännen asteen repeämät ovat harvinaisempia.

Episiotomia voidaan tehdä esimerkiksi silloin, jos kyseessä on suurikokoinen sikiö, voimakas arpikudos välilihalla, sikiön tarjontavirhe tai imukuppisynnytys.
Synnytysvauriot voidaan ommella monella eri tavalla. Ompelu on toimenpiteenä steriili. Yleensä ompelu tapahtuu absorboituvalla eli sulavalla ommelaineella. Synnytysvaurioiden ompelussa yleisimmin käytettäviä tekniikoita ovat katko-ommel ja jatkuvaommel. Lisäksi voidaan käyttää patjaommeltekniikkaa tai ihonalaiskudoksessa kulkevaa ommeltekniikkaa.
Opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä. Tämän opinnäytetyön tarkoituksena on lisätä kätilöiden tietoutta synnytysvaurioiden ompelussa käytettävistä ommeltekniikoista sekä tukea itsenäistä ompelutekniikoiden ja solmujen teon harjoittelua tuottamalla ompelutekniikoista opetusvideo. Tavoitteena oli lisätä synnytysvaurioiden ompelutaitoja sekä kehittää Turun ammattikorkeakoulun kätilöopiskelijoiden itseopiskelua ja kontaktituntien opetusta. Parempien ompelutaitojen avulla lisätään myös kätilöiden itsevarmuutta ja ammattitaitoa synnytysvaurioiden korjaamisessa sekä synnyttäjien normaalin anatomian palauttamisessa.
