Tiivistelmä
Ensihoidossa hengitystien hallinta on yksi hoidon kulmakivistä ja sen laiminlyönti voi johtaa jopa potilaan kuolemaan. I-gel on otettu käyttöön hengitystien hallintavälineenä Oulu-Koillismaan ja Jokilaaksojen pelastuslaitosten alueilla vuosien 2018–2019 aikana. Tämä opinnäytetyö on toteutettu yhteistyössä kyseisten pelastuslaitosten kanssa, ja sen tarkoituksena on kuvailla ensihoidossa toimivien ammattilaisten käyttökokemuksia i-gelin toimivuudesta hengitystien hallintavälineenä elottomilla potilailla. Opinnäytetyön tavoitteena on tuottaa tietoa i-gelistä ensihoidon käytössä. Tutkimuksen tuloksia voidaan hyödyntää arvioidessa i-gelin soveltuvuutta hengitystien hallintavälineenä sairaalan ulkopuolisessa ensihoidossa. Oulu-Koillismaan ja Jokilaaksojen pelastuslaitokset voivat tulosten perusteella arvioida i-gelin soveltuvuutta potilasturvallisuuden näkökulmasta, sekä ensihoitajien lisäkoulutuksen tarvetta i-gelin käyttöön liittyen.
Tietoperustassa käsittelemme hengitystien hallintaa ensihoidossa, hengitystien hallinnan indikaatioita, välineistöä, ja ensihoitajilta vaadittavia hoitovelvoitteita alan kirjallisuuden ja tutkimusten avulla. Tutkimusmenetelmänä oli kvantitatiivinen kyselytutkimus, jonka toteutimme Webropol-ohjelmalla. Vastauksia saimme yhteensä 62 kappaletta.
Tuloksista käy ilmi, ettei ensihoitajille ole ennättänyt kertyä vielä kovinkaan paljon käyttökokemuksia i-gelistä. Tulosten mukaan hengitystie on saatu yleensä hallintaan ensi yrittämällä ja lähes puolet tilanteista on sujunut ilman komplikaatioita. Ensihoitajien kokemuksen mukaan i-gel on pääsääntöisesti helppokäyttöinen hengitystien hallintaväline. Ensihoitajat kokevat osaavansa käyttää i-geliä ja suurin osa heistä ei koe tarvitsevansa lisäkoulutusta välineen käyttöön. Vastausten perusteella valtaosa ensihoitajista kokee i-gelin luotettavaksi hengitystien hallintavälineeksi. In the field of emergency nursing, airway management is one of the cornerstones of treatment and it can be performed using various methods. Prehospital airway management is one of the most demanding ja important procedures saving patients in out-of-hospital situations. Intubation is still considered the ‘gold standard’ in emergency airway management. For a long time, pro-fessionals have thought intubation is the best way to ensure sufficient ventilation ja oxygenation. However recently, there has been carried out multiple studies showing that intubation as a pro-cedure is rare and providers are not sufficiently experienced to execute it. Those studies also show that the new supraglottic airway management devices are easy and fast use, and they provide effective ventilation in both operating rooms and in out-of-hospital scenes.
I-gel has been introduced as an airway management device in the areas of the Oulu-Koillismaa and Jokilaakso rescue departments during 2018–2019. This thesis has been carried out in collaboration with these rescue departments and is intended to describe the experience of emergency professionals in the use of i-gel as an airway management device on lifeless patients. The aim of the thesis is to produce information about i-gel used by paramedics. The results of the study can be used to assess the suitability of i-gel as an airway management device in prehospital emergency care. Based on the results, the rescue departments of Oulu-Koillismaa and Jokilaakso can assess the suitability of i-gel from the perspective of patient safety, as well as the need for additional training for paramedics using the i-gel.
Our research method was quantitative survey, which we conducted with the Webropol-program. Our survey was sent all in all to about 350 paramedics and from all of those we got 62 answers back. Our knowledge base is extensive because recently there has been done a lot of studies regarding i-gel. We picked widely different studies around the world and they discuss airway management in emergency care, indications for airway management, equipment and care obli-gations required from paramedics.
The results of the study show that paramedics have not yet gained much experience using the i-gel. Our study also shows that paramedics have gotten patients airway secured on the first try using i-gel in most cases and almost half of the situations have gone without complications. Paramedics feel that they know how to use the device and most of them do not feel that they need additional training. In conclusion i-gel easy to use and based on our study, the majority of paramedics think that i-gel is a reliable airway management device.
