Tiivistelmä
Äidin psyykkisen hyvinvoinnin huomioiminen ja tukeminen kuuluvat synnytyksen jälkeiseen kätilötyöhön. Tämä tulee huomioida tasavertaisesti fyysisen toipumisen kanssa. Psyykkiseen hyvinvointiin postnataalivaiheessa vaikuttavat äidin elämäntilanne, kulttuuri, menneisyyden kokemukset, perinataalikauden aikaiset tapahtumat sekä synnytyskokemus. Postnataaliaika on erityisen herkkää aikaa äidin mielenterveyden kannalta. Olemassa oleva tai alkava mielenterveyden häiriö voi näyttäytyä ensimmäistä kertaa terveydenhuollon ammattilaiselle raskausajan seurannan tai synnytyksen jälkeisen hoitotyön aikana. Kätilön tehtävä on havainnoida, huomioida ja ottaa puheeksi äidin mieliala synnytyksen jälkeen.
Opinnäytetyö tuotettiin kehittämistyönä, jonka aiheena on äidin psyykkisen hyvinvoinnin huomioiminen ja tukeminen lapsivuodeaikana. Tilaaja on Kainuun keskussairaalan synnytysosasto 4, joka kuuluu Kainuun sosiaali- ja terveydenhuollon kuntayhtymään. Kehittämistyön tuotoksena syntyi tietopaketti kätilötyön tueksi. Aihe on rajattu äiteihin, joilla on takanaan säännöllinen raskausaika ja synnytys ilman komplikaatioita. Tietopaketin sisältöä voidaan hyödyntää myös näihin ryhmiin lukeutuvien äitien kanssa.
Kehittämistyön tarkoituksena oli tuottaa sisällöltään laadukas tutkimuksiin sekä muuhun teoreettiseen tietoon pohjautuva tietopaketti kätilöille äidin psyykkisen hyvinvoinnin huomioimiseksi ja tukemiseksi postnataaliaikana. Kehittämistyön tavoitteena on edistää kätilöiden valmiuksia havainnoida, tukea ja ottaa puheeksi äidin psyykkinen hyvinvointi. Tavoitteena on myös ylläpitää ja lisätä äidin sekä koko perheen mielenterveyttä. Tietopaketti sisältää teoriatietoa ja konkreettisia keinoja psyykkisen hyvinvoinnin huomioimiseksi ja tukemiseksi kätilötyössä.
Jatkokehittämisajatuksena esille nousi materiaalin tuottaminen äideille ja perheille. Materiaalissa käsiteltäisiin synnytyksen jälkeistä aikaa ja siihen liittyviä psyykkisen hyvinvoinnin haasteita. Materiaali jaettaisiin perheille jo äitiysneuvolassa. The consideration and support of the mother's psychological well-being is part of the postpartum midwifery. This should be taken into account on an equal footing with physical recovery. Mental well-being at the postnatal stage is influenced by the mother's life situation, culture, past experiences, perinatal events and child birth experience. The postpartum period is a particularly sensitive time for maternal mental health. An existing or emerging mental disorder may first appear to a health care professional during antenatal or postnatal care. The task of the midwife is to observe and bring up the mood of the mother after giving birth.
The thesis was carried out as a development work on the consideration and support of the mother's psychological well-being in the postpartum period. The client organization of thesis was Delivery Ward 4 at Kainuu Central Hospital that belongs to Kainuu Social Welfare and Health Care Joint Authority. As a result of the development work, a guideline was created to support the work of midwives. The topic is limited to the mothers who have a regular pregnancy and delivery without complications. The content of the guideline can also be used with mothers in these groups.
The purpose of the development work was to produce a high quality content based on research and other theoretical knowledge for midwives to consider and support maternal psychological well-being during the postnatal period. The goal of the development work is to enhance midwives' ability to observe, support and talk about mothers´ psychological well-being. The aim is also to maintain and increase the mental health of the mother and the family. The guideline contains theoretical knowledge and concrete ways to consider and support psychological well-being in midwifery work.
A topic for further development of the study came from producing material for mothers and families. The material would deal with the postnatal period and the related challenges of mental well-being. The material could be distributed to families already at the maternity clinic.
