Tiivistelmä
Sairaanhoitajien rajattu lääkkeenmäärääminen on yksi keino vapauttaa lääkärien työaikaa lääketieteen vaativampaa osaamista edellyttäviin tehtäviin, lyhentää potilasjonoja sekä lisätä asiantuntijasairaanhoitajien osaamista ja sen hyödyntämistä. Lääkkeitä määräävien sairaanhoitajien työnkuvasta ja ammattitaidon hyödyntämisestä Keski-Suomen hyvinvointialueella ei ole kokonaisvaltaista tietoa. Tämän opinnäytetyön tarkoituksena oli saada kokonaiskuva Keski-Suomen hyvinvointialueen lääkkeitä määräävien sairaanhoitajien osaamisen hyödyntämisen nykytilasta ja työtyytyväisyydestä sekä työtehtävien kehittämistarpeista.
Tutkimusaineisto kerättiin Webropol- kyselynä kesä- elokuun 2023 välisenä aikana Keski-Suomen hyvinvointialueella työskenteleviltä rajatun lääkkeenmääräämisoikeuden omaavilta sairaanhoitajilta (N=55). Tutkimuksessa käytettiin kyselylomaketta Finnish nurse prescribing questionnaire (FiNPQ). Kyseessä oli poikkileikkaustutkimus kokonaisotanalla.
Vastaajista (n=38) 45 % käytti työajastaan yli 60 % rajatun lääkkeenmääräämisen mahdollistaviin kliinisen hoitotyön tehtäviin. Suurin osa vastaajista (78 %, n= 31) teki päivittäin systemaattista anamneesia ja terveyshistorian kartoitusta. He tutkivat viikossa keskimäärin 31 potilasta ja hoitivat 41 potilasta. Vastaajista (n=41) 59 % antoi neuvontaa päivittäin lääkkeistä ja niiden sivuvaikutuksista. Uusia lääkemääräyksiä teki vastaajista päivittäin 7 % ja viikoittain suurin osa (59 %). Lääkitysten uusimisia olemassa olevan hoitosuunnitelman mukaisesti teki vastaajista viikoittain 7 % ja kuukausittain 22 %. Vastaajista 32 % ei ollut koskaan tehnyt olemassa olevan lääkemääräyksen uusimista. Rajatun lääkkeenmääräämisen piiriin kuuluvan lääkevalikoiman laajentamista toivoivat hiukan yli puolet vastaajista (53 %).
Tulokset osoittavat, että lääkkeitä määräävien sairaanhoitajien osaamista ei hyödynnetty parhaalla mahdollisella tavalla ja sitä voitaisiin parantaa monissa yksiköissä. Potilaiden ohjautumista sairaanhoitajien vastaanotolle tulisi tehostaa ja prosesseja tarkastella, jotta erityisosaaminen saataisiin hyödynnettyä. Lääkkeitä määräävien sairaanhoitajien työaikaa tulee kohdentaa lääkkeenmääräämisen työtehtäviin enemmän. Sähköisten palveluiden kehittäminen ja ottaminen osaksi lääkkeitä määräävien sairaanhoitajien työnkuvaa nykyaikaistaisi toimintaa ja parantaisi palvelujen saatavuutta.
Kehittämistarpeiksi tässä opinnäytetyössä nousivat lääkityksiin ja lääkemääräyksiin liittyvät uudistukset sekä kohdennettujen koulutusten mahdollistaminen säännöllisesti oman ammattitaidon ylläpitämisen varmistamiseksi.
The limited right of nurses to prescribe medication is one way to release doctors' time for tasks requiring demanding medical expertise, shorten patient queues, and increase utilization of nurses' advanced com-petence. Nurse prescribers job description and the utilization of their professional skills has not previously been investigated in the Wellbeing Services County of Central Finland. The purpose of this thesis was to find out how the competence of nurses who had completed limited prescribing training was utilized in the Wellbeing Services County of Central Finland and how nurses themselves perceived their own job de-scription. The thesis also examined how satisfied nurses were with their own job description and what kind of future development needs they felt there were in their work
The research material was collected as a Webropol survey between June and August 2023 from nurses with limited prescribing rights working in the Wellbeing Services County of Central Finland (N=55). The data was collected by Finnish nurse prescribing questionnaire (FiNPQ).
This was a cross-sectional study with an overall sample.
Of the respondents (n=38), 45 % spent over 60 % of their working time on clinical nursing tasks which en-able the nurses to use their limited right to prescribe medicines. Most of the respondent (78%, n=31) did a systematic anamnesis and health history mapping every day. Average of 31 patients were examined and 41 was treated during one week. Of the respondents 59 % (n=24) gave advice about medicines and their side effects on daily basis. New prescriptions were made daily by 7 % and weekly by 59 % of the respondents. Renewals of prescriptions according to the existing treatment plan were made weekly by 7 % and monthly by 59 % of the respondents. Of the respondents 32 % had never renewed prescriptions. A little more than half of the respondent (53 %) wished to expand the range of medicines that nurses with limited rights can prescribe.
The results show that the utilization of nurses’ competence in prescribing was not used in the best possi-ble way and could be improved in many units. The referral of patients to nurses' appointments should be made more efficient and processes should be reviewed in order to make the best use of special exper-tise. The working hours of nurse prescribers` should be allocated more to prescribing tasks. For the most part, the respondents were satisfied with their own job description, even though its utilization was limited. The development and inclusion of digital services as part of the job description of nurse prescribers` medicines would modernize operations and improve the availability of services.
The development needs in this thesis were reforms related to medication and prescriptions, as well as enabling targeted training on a regular basis to ensure the maintenance of nurse prescribers` professional skills.
