Tiivistelmä
Neurologiset sairaudet ovat hermoston ja lihaksiston sairauksia. Ne vaikuttavat henkilön toimintakykyä alentavasti muun muassa muistiin, kognitioon ja havaintokykyyn. Sairaudet ovat eteneviä, jolloin oirekuva monimuotoistuu ja toimintakyky laskee. Jossain vaiheessa sairautta on todennäköisesti tarvetta ympärivuorokautiselle palveluasumiselle.
Neuroliitto on järjestö, joka edistää ja valvoo MS-tautia tai harvinaista neurologista sairautta sairastavien henkilöiden ja heidän läheistensä etuja yhteiskunnassa. Neuvonnan lisäksi Neuroliitto tuottaa kuntoutus- ja asumispalveluja. Tehostettua palveluasumista tuotetaan neljässä palveluasumisen yksikössä. Uusin yksikkö, Kuntokoti, on aloittanut toimintansa syksyllä 2021 ja sen toimintaa lähdettiin tämän opinnäytetyön myötä kehittämään Lean-ajattelun avulla. Toiminnan kehittämiselle oli tarvetta, koska osin toimintatavat ja käytännöt olivat pirstaleisia, ja kaivattiin selkeytystä arjen hoitotyön organisointiin.
Opinnäytetyön tavoitteena oli kehittää hoitotyön arjen organisoinnin toimintatapoja Lean-ajattelua hyödyntäen, jotta työ olisi sujuvaa, tekijöille mielekästä ja tuottaisi asukkaille mahdollisimman paljon arvoa. Työn tarkoituksena oli luoda hoitotyön arjen johtamisen työkalu, joka helpottaa työnjakoa.
Opinnäytetyö oli tutkimuksellinen kehittämistyö, jossa käytettiin sekä laadullisia että määrällisiä menetelmiä. Kehittämistyötä tehtiin sekä Webropol-kyselyn että työpajatyöskentelyn keinoin. Kehittämistyöhön osallistui yksikön hoitohenkilökuntaa. Tutkimusaineistoa analysoitiin kuvailevan tilastoanalyysin, sisällönanalyysin sekä teemoittelun avulla.
Kehittämisen osalta opinnäytetyön tuloksena syntyi päivittäisjohtamisen taulu, Kanban-taulu, joka helpottaa Kuntokodin arjen hoitotyön organisointia.
Tulevaisuuden tutkimuskohteina voisi olla henkilökunnan näkemysten kartoittaminen taulun toimivuudesta sekä asukastyytyväisyyden mahdolliset muutokset taulun käyttöönoton jälkeen. Neurological diseases are diseases of the nervous and muscular systems. They have a debilitating effect on a person's ability to function, including memory, cognition and perception. The diseases are progressive, resulting in a complexity of symptoms and a decline in functional capacity. At some point in the disease, there is likely to be a need for 24-hour service accommodation.
Neuroliitto is an association that promotes and monitors the interests of people with MS or a rare neurological disease and their relatives in society. In addition to counselling, Neuroliitto provides rehabilitation and housing services. Intensified assisted living is produced in four assisted living units. The newest unit, Kuntokoti, started its operations in autumn 2021, and with this thesis, its operations were developed using Lean thinking. There was a need for operational development, as some of the operating methods and practices were fragmented, and there was a need for clarity in the organisation of everyday nursing work.
The aim of the thesis was to develop the operating methods of the everyday organization of nursing work using Lean thinking, so that the work would be smooth, meaningful for the workers and would produce as much value as possible for the residents. The purpose of the thesis was to create a tool for the daily management of nursing work that facilitates the division of labour.
The thesis was a research and development project using both qualitative and quantitative methods. The development work was carried out using both a Webropol survey and a workshop. The development work involved the nursing staff of the unit. The research material was analysed using descriptive statistical analysis, content analysis and thematic analysis.
As a result of the development, a daily management board, a Kanban board, was created, which facilitates the organisation of everyday nursing work in the Kuntokoti.
Future research could be carried out by the staff to explore the views of the staff on the functionality of the board and the possible changes in the satisfaction of the residents after the introduction of the board.
