Tiivistelmä
Palliatiivisella hoidolla tarkoitetaan oireenmukaista hoitoa, johon siirryttäessä sairauden ennusteeseen ei pystytä enää lääketieteen avulla vaikuttamaan. Palliatiivisen hoidon tarkoituksena on keskittyä asukkaan ja tämän omaisten kärsimyksen lievittämiseen ja se kuuluu kaikille, jotka sairastavat parantumatonta sairautta. Saattohoito on osa palliatiivista hoitoa, ja se kattaa kuolevan viimeiset elinpäivät tai –viikot.
Opinnäytetyö tehtiin tehostetun palveluasumisen yksikköön Attendo Männistön sydämeen, jonka saattohoitotoimintaa oli tavoitteena kehittää. Tämän lisäksi halusimme lisätä asukkaiden omaisten tietoisuutta ja osallistamista saattohoitoprosessiin. Opinnäytetyössä kuvataan saattohoitoa osana palliatiivista hoitoa, saattohoidossa olevan yleisimpiä oireita, kuten kipua, ja niiden arvioimista ja hoitamista, henkilökunnan osaamisen merkitystä sekä omaisten osallistamista ja tukemista.
Tämä kehittämistyö toteutettiin lineaarisen mallin avulla. Kehittämistyön lineaarinen malli koostuu suunnittelu-, toteutus- ja arviointivaiheesta. Tiedonhaussa käytimme sekä kansallisia, että kansainvälisiä tutkimusartikkeleja sekä muita luotettavia tiedonlähteitä, kuten Käypä hoito -suosituksia ja Sosiaali- ja terveysministeriön raportteja.
Palliatiivinen hoito sekä saattohoito koskettavat jokaista sosiaali- ja terveysalan ammattihenkilöä, ja palliatiivisen hoidon osaaminen kuuluu perusammattitaitoihin. Tämän hetken tiedon mukaan saattohoidon osaamisessa on Suomessa edelleen paljon puutteita, joten opinnäytetyön aihe on merkityksellinen. Opinnäytetyön tarkoituksena oli teoriatiedon pohjalta suunnitella, toteuttaa ja arvioida tehostetun palveluasumisen yksikön hoitajille sekä asukkaiden omaisille opas muistisairaan asukkaan saattohoidosta. Opas on toteutettu niin, että kokonaisrakenne on selkeä sekä helppolukuinen ja sen kielellinen ilmaisu on helposti ymmärrettävää. Palliative care means symptomatic treatments, when the prognosis of the disease is such that it is no longer possible to influence it with the help of medicine. The purpose of palliative care is to focus on alleviating the suffering of the resident and their relatives, and it belongs to everyone suffering from an incurable disease. End-of-life care is a part of palliative care, and it covers the last days or weeks of the dying resident’s life.
The thesis was written for the intensified service housing unit Attendo Männistön sydän, whose end-of-life care activities were to be developed. In addition, we wanted to increase the awareness and involvement of residents’ relatives in the end-of-life care process. The thesis describes end-of-life care as a part of palliative care, the more common symptoms of palliative care, such as pain and their assessment, the importance of staff competence, and the involvement and support of relatives.
This development work was carried out using a linear model. The linear model of development work consists of a planning, implementation, and evaluation phase. In our information search, we used both national and international research and other sources of information, such as Current Care Guidelines, and reports from the Ministry of Social Affairs and Health.
Palliative care and end-of-life care concern every social and health care professional, and palliative care competence is part of basic professional skills. According to current information, there are still many shortcomings in end-of-life care competence in Finland, so the topic of the thesis is relevant. The purpose of the thesis, based on theoretical knowledge, was to plan, implement and evaluate a hospice care guide for the nurses of an intensified assisted living unit and the residents' relatives on end-of-life care for a resident with a memory disorder. The guide has been designed in such a way that the overall structure is clear and easy to read, and its linguistic expression is easy to understand.
