Tiivistelmä
Rintasyöpä on naisten yleisin syöpä, joka todetaan Suomessa vuosittain noin 5 000 naisella. Rintasyövän esiintyvyys on viime vuosina lisääntynyt, mutta myös sen paranemisennuste on kasvanut. Rintasyöpähoitojen kehittymisen myötä sairastuneet voivat hoitojen jälkeen usein palata työelämään ja normaaliin arkeen. Rintasyöpähoitojen jälkeiset toimintahäiriöt ovat kuitenkin yleisiä ja voimakkaasti arkeen vaikuttavia haittatekijöitä.
Rintasyöpä hoidetaan leikkaushoidolla, johon yhdistetään lähes aina sädehoitoa sekä liitännäislääkehoitoa. Näiden hoitojen jälkeen ilmaantuvia yleisiä toimintahäiriöitä ovat tutkimusten mukaan leikatun puolen yläraajan turvotus, olkanivelen liikelaajuuden rajoittuminen sekä leikkausarpien kiristyminen. Haittavaikutusten ennaltaehkäisy ja kuntouttaminen rintasyöpähoitojen jälkeen on tärkeää sairastuneen toimintakyvyn sekä elämänlaadun parantamiseksi.
Opinnäytetyön tarkoituksena oli tuottaa opas rintasyöpähoitojen jälkeisten yläraajan toimintahäiriöiden kuntoutuksesta. Tavoitteena oli koota toimeksiantajalle tietoa erilaisten fysioterapeuttisten keinojen vaikuttavuudesta rintasyöpähoitojen jälkeisten yläraajan toimintahäiriöiden kuntoutuksessa sekä tarjota konkreettinen työväline tukemaan rintasyövän sairastaneen asiakkaan kuntoutusta. Opinnäytetyö toteutettiin tuotekehitysprosessina. Työ sisältää teoreettisen viitekehyksen, joka koostuu rintasyövän hoidosta ja sen jälkeisistä yläraajan toimintahäiriöistä sekä niiden kuntoutuksesta. Opinnäytetyön aihe on rajattu rintasyöpähoitojen jälkeisiin yleisimpiin yläraajojen toimintahäiriöihin.
Opinnäytetyössä käsiteltävät kuntoutusmenetelmät on rajattu tuoreimpien tutkimusten perusteella tehokkaimpiin menetelmiin. Lymfaturvotuksen, olkanivelen liikerajoitteiden ja arpien kiristymisen ennaltaehkäisyyn sekä kuntoutukseen tehokkaita keinoja ovat lymfakäsittelyt sekä faskiakalvojen ja arpien käsittely. Terapeuttisen harjoittelun, joka sisältää liikkuvuus- ja voimaharjoittelua, on todettu sekä ehkäisevän toimintahäiriöitä että lievittävän jo ilmaantuneita liikerajoituksia. Edellä mainittujen fysioterapeuttisten menetelmien on myös todettu lievittävän kipuoireita leikkausalueella sekä parantavan elämänlaatua. Yläraajan toimintahäiriöiden kuntoutuksessa on todettu tehokkaaksi mainittujen terapiamenetelmien yhdistelmät, mutta yksittäisten menetelmien vaikutuksista tarvitaan lisää tutkimuksia. Breast cancer is the most common cancer in women - it is diagnosed annually in 5 000 women in Finland. The incidence of breast cancer has increased in recent years, but so has the prognosis for its cure. With the improvement of breast cancer treatments most patients can return to work and daily life. However, dysfunctions in upper limbs are common after breast cancer treatment and can have a strong impact on everyday life.
Breast cancer is treated with surgery, usually combined with radiotherapy and complementary medication. According to studies, common dysfunctions that appear after treatments are swelling of the upper limb, limitation in the range of motion of the shoulder joint and tightening of surgical scars. Prevention and rehabilitation of these dysfunctions is important to improve the patient’s functional capacity and quality of life.
The objective of the thesis was to produce a written guide on the rehabilitation of upper limb dysfunctions after breast cancer treatments. The goal was to provide the commissioner with information on the dysfunctions and the effectiveness of different physiotherapy methods treating them, as well as a concrete tool for sharing this information with clients. The study was implemented as a product development process. The work includes a theoretical reference framework consisting of breast cancer treatment and subsequent upper limb dysfunctions and their rehabilitation. The topic of the thesis is limited to the most common dysfunctions of the upper limb after breast cancer treatments.
The rehabilitation methods discussed in the thesis have been limited to the ones that have been proven most effective according to recent studies. Based on research, effective ways to prevent and rehabilitate lymphedema, movement restrictions of the shoulder joint and tightening of scars are lymphatic drainage and the mobilization of fascia and scar tissue. Therapeutic exercise, which includes mobility and strength training, has been found to both prevent functional disorders and relieve movement restrictions that have already appeared. The aforementioned physiotherapy methods have also been found to relieve pain symptoms in the surgical area and improve quality of life. Combination of these methods seems to be effective, but more research could be done regarding the use of physiotherapeutic methods individually.
