Tiivistelmä
Opinnäytetyön tarkoituksena oli kartoittaa saattohoitopotilaiden henkisen tuen tarvetta. Tavoitteena on tuottaa tietoa, jota voidaan hyödyntää moniammatillisessa hoitotyössä erityisesti saattohoitopotilaan sielunhoitoon liittyvissä kohtaamisissa.
Opinnäytetyö on laadullinen tutkimus, jossa kerättiin kaupunginsairaalan saattohoito-osastolta kaksi aineistoa: saattohoito-osastolle tulevien potilaiden tulohaastattelulomakkeiden vastaukset (n=36) ja sairaanhoitajien ryhmähaastattelu (n=5). Haastattelut litteroitiin ja aineisto analysoitiin induktiivisesti laadullisella sisällönanalyysillä.
Tutkimustulosten perusteella voidaan todeta, että tulohaastattelulomakkeen käyttö on tärkeää iäkkäiden pitkäaikaishoitoa saavien henkilöiden elämän loppuvaiheen hoidon ja huolenpidon turvaamiseksi. Lomakkeen tiedoilla voidaan saada tietoa potilaan toiveista ja peloista sekä millaiset ovat voimaannuttavia ja iloa tuovia asioita. Hoitosuunnitelmaan tulohaastattelussa ei lomakkeeseen kuitenkaan kirjaudu kaikkea mitä potilas haluaa tai kaipaa ja siksi läsnäolo, kuunteleminen ja potilaan voinnin seuraaminen on tärkeää.
Vastausten perusteella voi päätellä, että potilaat ovat ymmärtäneet, mitä saattohoito-osastolle tuleminen tarkoittaa, sillä vastauksissa ei ollut toiveita parantumisesta, arjen asioiden hoitamisesta tai kotiin lähtemisestä.
Potilaiden pelot ja toiveet liittyivät kipuun. Kipua pelättiin ja samalla toivottiin hyvää kivun lievitystä. Pelon aiheita olivat myös turvattomuuden tunne, yksin jääminen kuoleman hetkellä ja kuoleman pelko.
Potilaiden toivoa vahvistavat tekijät liittyvät jokapäiväiseen elämään sekä kuolemaan valmistautumiseen.
Tärkeitä iloa ja tukea sekä voimaannuttavia sairaudessaan ja arjen elämään liittyviä asioita olivat perhe, läheiset ja harrastukset. Potilaat toivoivat omaa rauhaa ja hiljaisuutta heidän toiveitaan kuunnellen ja ymmärtäen sekä turvallista ja hyvää hoitoa. Yli puolet potilaista ei kokenut tarpeellisena tavata sosiaalityöntekijää, sairaalapastoria, vapaaehtoistyöntekijää eikä osallistua hartaushetkiin.
Hoitajat kokivat moniammatillisen tiimin laaja-alaiseksi asiantuntijaryhmäksi, joka ei rajoitu varsinaista hoitotyötä tekeviin professioihin, vaan tiimiin kuuluvat myös esimerkiksi laitosapulaiset. Myös syöpäyhdistyksen kouluttamien vapaaehtoisten toimintaa pidettiin tärkeänä.
Hoitajien mielestä henkinen tuki on olennainen, hoidon arkeen kuuluva osa saattohoitoa. Hoitajien rohkeus ja omatoimisuus ovat tärkeitä ominaisuuksia henkisen tuen antamisessa. Sairaalapastorit on ollut hoitajien kokemusten mukaan helppo tavoittaa. Hoitajat kaipasivat erityisesti kuoleman kohtaamiseen ja monikulttuurisuuteen liittyvää koulutusta. The purpose of this thesis was to assess the need for pastoral care for hospice patients., with the aim of produceing information that can be utilised in multiprofessional nursing, and especially in encounters related to the pastoral care of patients with hospice care.
In this qualitative study two datasets were collected from the hospice department of a city hospital: responses to initial interview forms filled in when the patients (n=36) entered the hospice ward and
responses obtained in a group interview with nurses (n=5). The interviews were transcribed and the data processed inductively by qualitative content analysis.
It can be concluded from the results that the initial interview form is important for ensuring adequate long-term care for the elderly and proper treatment for the last stage in their lives. The information can be used to formulate a picture of the patient's hopes and fears and what things bring empowerment and joy. The form does not, however, contain everything that the patient wants or needs, and therefore it is important for someone to be present, to listen and to monitor the patient's condition.
It can be concluded from the responses that the patients had understood what it means to come to the
hospice ward, as the transcriptions contained no hopes of recovery, of settling everyday matters or of going home.
The patients’ hopes and fears were associated with pain. They were afraid of pain and at the same time hoped for good pain relief. Fears were also aroused by feelings of insecurity, being left alone at the moment of death and the prospect of death itself.
Factors that gave the patients hope awere associated with everyday life and with preparation for death.
Important sources of joys and support, and of empowerment in the face of illness and everyday life, were the patient’s family, loved ones and hobbies. The patients hoped for peace and quiet, that their wishes would be listened to and understood, and that they would be in safe hands and receive good treatment. More than half of the patients did not feel any need to meet with a social worker, hospital pastor, or
volunteer, or to attend any religious service.
The nurses felt that their multiprofessional team was a wide-ranging group of experts that was not limited to professionals engaged in actual nursing but also included auxiliary staff, for example. The activities of
volunteers trained by the Cancer Society of Finland were also considered important.
The nurses believed emotional support to be an integral part of everyday care for patients, and regarded courage and initiative as important qualities for caregivers when providing this emotional support. In their experience it is easy to approach a hospital pastor. The nurses especially needed training in handling death and in the problems of a multicultural society.
