Lasten oikeuksien toteutuminen sairaalassa : kuvaileva kirjallisuuskatsaus, artikkeli-opinnäytetyö

Tiivistelmä
Lasten oikeuksia säädetään kansainvälisesti YK:n lasten oikeuksien sopimuksessa sekä Pohjoismaissa NOBAB standardeja noudattaen. Tämän kirjallisuuskatsauksen tarkoituksena oli kuvata, miten lasten oikeuksia toteutetaan sairaalassa. Opinnäytetyön tavoite on lisätä tietoa lasten oikeuksista hoitotyössä ja kehittää lasten hoitotyön käytäntöjä niin, että lasten oikeuksia toteutettaisiin mahdollisimman laajasti sairaalaympäristössä.
Artikkelimuotoinen opinnäytetyö toteutettiin kirjallisuuskatsauksena, johon kerättiin aineistoa Cinahl, Medic ja PubMed -tietokannoista. Tämän lisäksi materiaalia löytyi myös manuaalisen haun kautta. Lopulliseen aineistoon valikoitui kahdeksan tutkimusta, joiden tulokset analysoitiin induktiivisella sisällönanalyysillä.
Tulosten mukaan lasten oikeuksien toteutumiseen vaikuttivat tilojen sopivuus, perheiden ja potilaiden tiedonsaanti, hoitohenkilökunta, hoidon perhekeskeisyys sekä lasten huomioiminen. Sairaalan tilat olivat liian ahtaita. Tiedonkulkuun ja vanhempien tietoisuuteen lasten oikeuksista tulisi kiinnittää huomiota. Sairaanhoitajat kaipasivat lisää aikaa lapsipotilaiden kohtaamiseen ja heidän koulutustaan aiheesta olisi lisättävä. Vanhempien läsnäolon hoidon aikana turvattiin yleensä hyvin, mutta muun perheen huomioimisessa oli parannettavaa. Lasten oikeus kivunhoitoon ja itsemääräämisoikeuteen ei aina toteutunut toivotulla tavalla. Children’s rights are recognized internationally in the UN Convention of the Rights of the Child and in the Nordic countries in compliance with NOBAB standards. The purpose of the thesis was to describe how children’s rights are implemented in hospitals and its goal is to increase knowledge about children’s rights in nursing care and to develop paediatric nursing practices so that children’s rights are implemented as widely as possible in hospitals.
The literature review was developed using material from the Cinahl, Medic and PubMed databases. In addition, material was also found using manual search methods. Eight studies were selected and the results were analyzed using inductive content analysis.
The results showed that suitability of facilities, families’ access to information, staff, family-centeredness of care and consideration of children influenced the implementation of children’s rights. The hospital facilities were not spacious enough. Attention should be paid to the flow of information and parents’ awareness of children’s rights. Nurses needed more time to properly care for their patients and their training on the subject should be increased. Parents’ presence during treatment was generally well secured but there was room for improvement in the consideration of the rest of the family. Children’s rights to pain management and self-determination were not always fulfilled as hoped.
