Tiivistelmä
Botuliinitoksiinin ja hyaluronihapon käyttö on lisääntymässä lääketieteestä ja kauneudenhoidosta myös suun terveydenhoidon puolelle. Botuliinitoksiinipistoksia on tutkittu tähän mennessä hoitomuotona bruksismiin, kun taas hyaluronihappoa on hyödynnetty parodontologisessa hoidossa sekä suuhygieniatuotteissa. Menetelmät ovat kuitenkin kohtuullisen tuoreita, eikä niistä ole juurikaan saatavilla suomenkielistä tutkittua tietoa suunterveyden näkökulmasta. Tämän kuvailevan kirjallisuuskatsauksen tarkoituksena oli kuvata botuliinitoksiinin ja hyaluronihapon käyttöä suun terveydenhoidossa. Tavoitteena oli lisätä tietoa näiden menetelmien käytöstä suun terveydenhoidon opiskelijoiden ja ammattilaisten keskuudessa.
Kirjallisuuskatsauksen tilaaja oli Kuopion yliopistollinen sairaala KYS. Aineiston hankinta toteutettiin PubMed ja Cinahl tietokannoista ja tutkimukset valittiin määrittämillämme sisäänotto- ja poissulkukriteereillä. Kirjallisuuskatsaukseen valikoitui 40 kansainvälistä tutkimusta, joiden laadun arviointi tehtiin Hawkerin arviointityökalua hyödyntämällä. Kirjallisuuskatsaukseen hyväksyttiin 34 tutkimusta. Aineiston analyysi suoritettiin sisällönanalyysin menetelmän mukaisesti.
Tutkimustulokset osoittivat, että botuliinitoksiinilla ja hyaluronihapolla on useita käyttöaiheita suunterveydenhoidossa. Botuliinitoksiinilla on hoidettu onnistuneesti purentafysiologisia vaivoja, korkeaa ienhymyä sekä kasvojen alueiden kiputiloja. Hyaluronihappo edistää kudosten ja solujen uusiutumista, minkä vuoksi sitä käytetään parodontologiassa, ienkudoksen paranemiprosessissa, purentaan liittyvissä vaivoissa ja kiputiloissa. Molempia menetelmiä käytetään laajalti myös esteettisissä suun hoidon toimenpiteissä.
Tutkimustuloksista on hyötyä suun terveydenhoitoalalla opiskeleville ja työskenteleville, sillä riittävällä tutkimustiedolla voidaan kehittää uusia ja tehokkaita hoitomenetelmiä sekä lisätä alan ammattilaisten osaamista. Hoitojen lisääntyessä niitä voidaan tuoda myös suuremmalle ihmisjoukolle yksityissektorilta julkiselle puolelle potilaiden tarpeisiin. Jatkotutkimuksia botuliinitoksiinin ja hyaluronihapon käytöstä ja niiden vaikutuksista suuremmilla otannoilla kaivataan, jotta käyttöä olisi täysin turvallista laajentaa suun terveydessä. The use of botulinum toxin and hyaluronic acid is increasingly spreading from medic and cosmetic purposes to oral health care. Thus far, botulinum toxin injections have been studied in the treatment of bruxism, whereas hyaluronic acid has been used in periodontology and oral health products. However, these treatment methods are moderately new in oral health care, and therefore there is little research available about them in Finnish from the perspective of oral health. The purpose of this narrative literature review was to describe the use of botulinum toxin and hyaluronic acid in oral health care and the aim was to make the use of these methods more widely known among oral health students and professionals.
The client organization of this thesis was Kuopio University Hospital. The material for the narrative literature review was acquired from PubMed and Cinahl databases using the defined inclusion and exclusion criteria. 40 international studies were identified for this literature review and the quality assessment for these studies was done by using Hawker’s assessment tool. After that, 34 international studies were accepted for this review. The studies were analyzed using content analysis.
The results showed that there are many different uses for botulinum toxin and hyaluronic acid in oral health care. For instance, botulinum toxin injections have been effectively used to treat temporomandibular disorders, excessive gingival displays and orofacial pain disorders. Additionally, hyaluronic acid improves tissue and cell regeneration, which is why it has been used in periodontology and to enhance gingival wound healing and to treat temporomandibular disorders and pain. Both of these methods are also widely used in dental esthetics.
To conclude, the results are beneficial for oral health care professionals and students, because with enough research, new and efficient treatment methods can be developed and the expertise of the industry professionals improved even further. As these new treatments become more widely available, they can also be accessed by a larger number of patients in the public sector in addition to the smaller population of the private sector, and therefore the industry can strive to meet the needs of a greater patient population. However, further studies on the use of botulinum toxin and hyaluronic acid and their effects in larger sampling sizes are needed to expand the use of these methods safely in oral health.
