Tiivistelmä
Tämän kehittämistyönä toteutettavan opinnäytetyön tarkoituksena oli suunnitella, toteuttaa ja arvioida koulutusinterventio hoitohenkilökunnan palliatiivisen osaamisen vahvistamiseksi. Kehittämistyön tavoitteena oli edistää laadukkaan ja turvallisen palliatiivisen hoitotyön toteutumista Iin kotihoidossa ja kotisairaalassa.
Palliatiivisen hoidon osaaminen on noussut yhdeksi keskeisimmistä kehittämiskohteista. Varsinkin kotihoidon henkilökunnan osaaminen kivunhoidon, psykososiaalisen tuen ja saattohoidon osalta on noussut esille tehdyissä tutkimuksissa. Osaavaa henkilökuntaa tarvitaan, että voidaan toteuttaa laadukasta ja asiakaslähtöistä palliatiivista hoitoa asiakkaille siellä, missä he haluavat loppu elämänsä viettää.
Toimeksiantajana kehittämistyössä oli Oulunkaaren kuntayhtymän Iin kotihoito ja kotisairaala. Palliatiivisen hoidon tarve on kasvanut myös Iissä ja osaavaa henkilökuntaa tarvitaan toteuttamaan palliatiivista hoitoa ja saattohoitoa asiakkaiden kotiin.
Opinnäytetyö toteutettiin kehittämistyönä, jonka menetelmäksi valikoitui konstruktivistinen kehittämistyön malli. Koulutusinterventio toteutettiin Learning café -menetelmän avulla, jonka keskusteltavat aiheet valittiin Sosiaali- ja terveysministeriön määrittelemistä laatukriteereistä iäkkäiden sosiaalihuollon palliatiivisen hoidon palveluille.
Koulutusintervention oli kokonaisuudessaan onnistunut kokonaisuus ja osallistujat kokivat sen hyväksi oppimistavaksi. Osallistujien mielestä Learning café sopii hyvin oppimisen työvälineeksi, mutta hieman heikommin se sopi palliatiivisen hoidon opettamiseen.
Jatkossa koulutusinterventiota voi kehittää verkossa tapahtuvaksi, jotta saavutetaan enemmän hoitohenkilökuntaa. Sekä aiheet voidaan valita kehittämään muita palliatiivisen hoidon osa-alueita tai muita hoidollisia asioita, kuten muistin tukeminen. Jatkossa myös Iin kotihoidossa ja kotisairaalassa palliatiivisen hoidon osaamista voidaan kehittää PACE-toimintamallin avulla. Abstract
The aim of this thesis was to plan, implement and evaluate an educational intervention to strengthen nursing staff's palliative skills in order to promote high-quality and safe palliative care in home care and at-home hospital of the client organisation.
Expertise in palliative care has become one of the most important areas for development in recent years. In particular, the expertise of home care staff, in terms of pain management, psychosocial support and hospice care, has come to the fore in various studies. Competent and highly skilled staff is needed in order to deliver high-quality and patient-centred palliative care to ensure that patients are entitled to have a choice about where they wish to spend the last months of their lives.
The thesis was commissioned by Ii home care and at-home hospital of Oulunkaari Joint Municipal Authority. The need for palliative care has grown in Ii, which means skilled staff are needed to provide palliative care and hospice care at the patients' homes.
The method chosen for the thesis is the constructivist model of development work. The educational intervention was implemented using the Learning Café method, and the topics discussed were selected from the quality criteria defined by the Ministry of Social Affairs and Health for palliative care services for the elderly.
The educational intervention was a successful entity and the participants felt it was a good way to learn. According to the participants, the Learning Café model is well suited as a learning tool. However, it was a little less suitable for teaching palliative care as a concept.
In the future, this type of educational intervention can be developed to take place online in order to reach more nursing staff. The topics can be chosen to develop other aspects of palliative care or other nursing issues, such as training memory support. In the future, palliative care skills can also be developed in the home care and at-home hospital of Ii with the help of the PACE operating model.
