Tiivistelmä
Terveydenhuollossa tapahtuu usein virheitä, jotka voidaan estää monilla ennakoivilla keinoilla. Kliinisessä laboratoriossa virheitä ja niistä aiheutuvia vaaratapahtumia sekä poikkeamia tapahtuu eniten preanalytiikan vaiheessa. Preanalytiikkaan kuuluvat kaikki ne vaiheet, jotka toteutetaan ennen kuin näyte saapuu mittaavalle laitteelle. Vaaratapahtumien ja poikkeamien kirjaaminen ja niistä raportointi on tärkeää. Näin voimme oppia jatkossa vastaavista tilanteista ja tapahtumista ja niiden toistumista voidaan ehkäistä.
Tämän opinnäytetyön tavoitteena oli kehittää Terveystalon potilasturvallisuutta parantamalla kliinisen laboratoriotyön laatua. Opinnäytetyössä tarkastellaan, kuinka paljon ja millaisia kliinisen laboratoriotoiminnan preanalytiikassa tapahtuneita vaaratapahtumia kirjataan Terveystalon vaaratapahtumajärjestelmään. Opinnäytetyön tarkoituksena oli Terveystalon työntekijöiden havaitsemien kliinisen laboratoriotyön preanalytiikan poikkeamien kirjaamisaktiivisuuden parantaminen.
Opinnäytetyössä käytettiin kvalitatiivista eli laadullista tutkimusotetta ja tutkimuskysymykset muotoiltiin henkilöstön sekä potilasturvallisuuden näkökulma huomioon ottaen. Opinnäytetyön avulla pyrittiin tuottamaan tietoa, jota voidaan hyödyntää jatkossa kehittämistoiminnassa. Tutkimusaineisto saatiin Terveystalon vaaratapahtumajärjestelmästä sekä laboratoriossa työskenteleville suunnatun kyselyn avulla. Aineisto analysoitiin käyttäen sisällön erittelyä sekä aineistolähtöistä sisällönanalyysiä.
Tulosten mukaan Terveystalon kliinisen laboratorion preanalytiikassa vuosina 2020–2021 tapahtuneista luokitelluista vaaratapahtumista eniten vaaratapahtumia ja niistä aiheutuneita haittatapahtumia oli aiheuttanut laboratorionäytteen katoaminen. Vuosien 2020–2021 aikana kaikista preanalytiikassa tapahtuneista vaaratapahtumista haittatapahtumia oli kirjattu enemmän (66 %) kuin läheltä piti -tapahtumia (33 %).
Terveystalon kliinisen laboratorion preanalytiikan poikkeamien ja vaaratapahtumien kirjaamisprosessin kehittämissuosituksina voidaan tulosten mukaan esittää poikkeamien kirjaamisen koulutuksen ja perehdytyksen lisääminen työntekijöille, poikkeamien kirjaamisprosessin ohjeiden selkeyttäminen, ilmoittamislomakkeen yksinkertaistaminen sekä työajan resursoiminen poikkeamien kirjaamista varten. There are typically mistakes in health care that can be prevented by a great number of proactive means. In the clinical laboratory, errors, incidents and deviations occur most frequently in the pre-analytical phase. More specifically, pre-analytics includes all the steps that are performed before the sample arrives at the measuring device. It is important to record and report incidents and deviations in the laboratory. In this way we can learn from similar situations and events in the future and prevent their recurrence.
The primary aim of this thesis project was to develop Terveystalo’s patient safety by improving the quality of clinical laboratory work. Hence, the thesis examines how many and what kind of incidents that have occurred in the pre-analytics of clinical laboratory operations are recorded in Terveystalo’s incident system. Importantly, the purpose of the thesis was to improve the recording activity of pre-analytical deviations in clinical laboratory work observed by Terveystalo’s employees.
The qualitative research approach was applied in the thesis and the research questions were approached from the perspective of the staff and patient safety. In view of this, the aim was to produce information that could be used in future development activities. The research material was obtained from Terveystalo’s incident system and through a survey targeted at those working in the laboratory. The data were analyzed using content analysis and data-driven content analysis.
In conclusion, according to the results, the largest number of classified incidents and adverse events in the pre-analysis of Terveystalo’s clinical laboratory in 2020–2021 had been caused by the disappearance of the laboratory sample. Moreover, during 2020–2021 more adverse events (66 %) than near misses (33 %) were recorded out of all incidents of pre-analysis.
As indicated by the results, the development of the process for recording deviations and incidents in the pre-analytics of Terveystalo’s clinical laboratory can be increased by training and familiarizing employees with the registration of deviations, clarifying the instructions for the prosses of recording deviations, simplifying the reporting form and rescheduling working time.
