Tiivistelmä
Opinnäytetyö käsittelee painehaavojen ennaltaehkäisyä. Opinnäytetyö tehtiin Attendo Pekkalakoti Oy:n omistaman hoivakoti Tikankontin toimeksiannosta. Aihe valittiin toimeksiantajan huolesta painehaavojen lisääntymisen myötä. Opinnäytetyön tarkoituksena oli tehdä näyttöön perustuva painehaavojen ennaltaehkäisyn toimintaohje hoivakoti Tikankonttiin. Tavoitteena oli, että hoivakoti Tikankontin hoitohenkilökunta tunnistavaa painehaavariskissä olevat asukkaat ja osaavat toiminnallaan ehkäistä painehaavoja. Teoriaosuus käsittelee riskikartoituksen ja painehaavojen ennaltaehkäisyn osa-alueita. Käsitteet olivat tärkeimmät teoreettiset tiedot opinnäytetyön tekemisessä. Lisäksi työssä käsiteltiin kirjaamista ja haittatapahtumailmoituksen tekemistä.
Opinnäytetyö tehtiin Jämsä-Mannisen tuotteistamisprosessin vaiheiden mukaisesti. Tuotteen tiedot koostettiin näyttöön perustuvista tiedoista ja suosituksista. Tuotteen toimivuutta koekäytettiin kuukauden ajan hoitohenkilökunnalla hoivakoti Tikankontissa. Koekäytön jälkeen tuotetta arvioitiin hoitohenkilökunnalta kerätyllä palautelomakkeella. Palautelomakkeeseen vastasi 8 työntekijää. Palautteiden perustella tuotetta viimeisteltiin. Luotettavuuden kriteereinä käytettiin laadullisen tutkimusprosessin menetelmiä soveltaen.
Tulokseksi saatiin painehaavojen ennaltaehkäisyn toimintaohje hoitohenkilökunnalle. Toimintaohjeessa kävi ilmi, miten tunnistetaan painehaavariskissä olevat asukkaat ja mitä asioita huomioidaan painehaavojen ennaltaehkäisyssä. Palautteiden antajat olivat tyytyväisiä tuotteen selkeyteen ja ymmärrettävyyteen sekä informatiivisuuteen. Palautteiden antajat olivat sitä mieltä, että tuotteesta on hyötyä heidän työssään jatkossa ja he olivat oppineet jotakin uutta painehaavojen ennaltaehkäisystä. Palautteessa ei noussut esille korjausehdotuksia. Palautteissa oli mainittu ravitsemuksen ja asentohoidon hyvät ohjeistukset. Lisäksi oli mainittu tuotteen herättelevän miettimään työntekijän omaa työskentelyä ja kiinnittämään siihen huomiota. Vähäisistä vastauksista huolimatta, vastausten yhteneväisyys puoltaa toimintaohjeen toimivuutta. Palautteiden mukaan voidaan todeta, että toimintaohje täytti tavoitteet.
Työn tuotos hyödyttää toimeksiantajaa. Toimintaohjeen avulla hoitohenkilökunta pystyy ennaltaehkäisemään painehaavoja tehokkaammin ja heidän toimintansa on yhtenäisempää. Jatkokehittämisideana voisi olla selvitys siitä, onko painehaavojen ennaltaehkäisyn toimintaohje helpottanut hoitohenkilökunnan toimintaa ja ovatko painehaavat vähentyneet yksikössä. Toinen jatkokehittämisen kohde voisi olla asukkaan yksilöllinen painehaavojen ennaltaehkäisyn suunnitelma, jossa painehaavariskissä olevalle asukkaalle laaditaan henkilökohtainen ennaltaehkäisevä hoitosuunnitelma. This thesis deals with the prevention of pressure sores and was made on the commission of Tikankontti Care Home, owned by Attendo Pekkalakoti Oy. The subject was chosen out of concern by the commissioner as pressure sores have become more common. The purpose of this thesis was to compile evidence-based guidelines for preventing pressure sores for Tikankontti Care Home. The aim was that nursing staff at the care home could identify residents at risk of developing pressure sores and know how to prevent pressure sores. The theory section discusses the areas of risk mapping and pressure sore prevention. The concepts were the main theoretical data in this thesis. In addition, documentation and making adverse event reports are addressed.
The thesis was conducted according to the steps of the Jämsä-Manninen productization process. Product information consisted of evidence-based information and recommendations. The functionality of the product was tested by nursing staff at Tikankontti Care Home during a one-month period. After trial use, the nursing staff evaluated the product using a feedback form. 8 employees filled in the feedback form. The product was finalized after feedback. Methods of the qualitative research process were applied as the criteria for reliability.
The result was a the Pressure Ulcer Prevention Action Guide for Nursing Staff. The activity guide showed how to identify residents at risk for pressure ulcers and what things to consider when preventing pressure ulcers. Feedback providers were satisfied with the clarity, comprehensibility and informativeness of tea in the product. Feedback providers believed the product would be useful in their work in the future and had learned something new about pressure ulcer prevention. There were no suggestions for a correction in the feedback. Workers had cited good guidelines for nutrition and posture therapy. It was also mentioned that the product attracted the thought of the employee's own work and attention to it. Despite the low number of responses, the commonality of responses advocates the functionality of the Code of Operations. According to feedback, it can be noted that the operating instruction met the objectives.
The output of the work benefits the subscriber of the work. With the help of the operating instructions, Nursing staff are able to prevent pressure sores more effectively and with it their activities more uniform. Further research could include a study of whether the Pressure Ulcer Prevention Guideline has helped the functioning of medical staff, as well as whether pressure sores have decreased in care home Tikankontti. Another further research could be a resident's individual pressure ulcer prevention plan, in which a personalized preventive care plan is developed for a resident at pressure sores risk.
