Tiivistelmä
Opinnäytetyön tavoitteena oli kehittää tehostetun palveluasumisen työyksikön toimintaa asukaslähtöisemmäksi ja kokonaisvaltaisemmaksi. Jotta tavoitteessa onnistuttaisiin, kehitettiin toimintayksikköön asukaslähtöinen omahoitajuuden toimintamalli prosessikuvauksena omahoitajuuden työvälineeksi. Opinnäytetyö tehtiin toimintatutkimuksena, johon sisältyi kolme vaihetta.
Toimintatutkimuksen ensimmäisessä vaiheessa tehtiin työntilaajan toimintayksikköön sähköinen omahoitajuuden kyselytutkimus, jossa haluttiin selvittää, mitkä ovat omahoitajuuden kehittämiskohteita tehostetun palveluasumisen työyksikössä. Kyselytutkimuksen kysymykset nousivat opinnäytetyön teoreettisesta viitekehyksestä. Kyselytutkimus lähetettiin 16 omahoitajalle, ja kyselyyn vastasi 11 omahoitajaa.
Toimintatutkimuksen toisessa vaiheessa haluttiin saada tieto siitä, mitä asukaslähtöinen omahoitajuuden toimintamalli sisältää. Kysymykseen vastausta lähdettiin etsimään työntilaajan omahoitajille pidettävästä kehittämisiltapäivästä, johon osallistui 13 omahoitajaa. Kehittämisiltapäivä pidettiin Learning Cafe -menetelmällä. Learning Cafen pöytäkuntiin nostettiin teemat opinnäytetyön suunnitelmavaiheen kyselytutkimuksesta teemoittelun avulla.
Opinnäytetyön teoreettisen viitekehyksen, kyselytutkimuksen ja kehittämisiltapäivän vastauksien perusteella laadin asukaslähtöisen omahoitajuuden toimintamallin tehostettuun palveluasumiseen prosessikuvauksena JHS 152. Prosessikuvauksessa tulee ilmi prosessien väliset riippuvuudet, vuorovaikutus ja prosessien tulokset.
Toimintamalli esiteltiin laadinnan jälkeen työyhteisöille. Toimintamallin toimivuutta ja mahdollisia kehittämistarpeita haluttiin selvittää palautekyselyn avulla. Palautekysely lähetetiin 19 omahoitajalle, kyselyyn vastasi 11 omahoitajaa. Palautekyselyn tuloksien perusteella asukaslähtöiseen omahoitajatoimintamalliin oltiin tyytyväisiä. Valtaosa vastaajista piti toimintamallia selkeänä, käyttökelpoisena ja asukaslähtöisenä. Valtaosa koki toimintamallin vastaavan kehitystarpeeseen. Kaikki vastaajat kokivat toimintamallin auttavan hahmottamaan omahoitajan työtehtäviä. Toimintamalli todettiin palautekyselyssä toimivaksi, eikä toimintamalliin tehty muutoksia. The aim of this thesis was to develop the operation of a 24-hour service house to be more customer oriented and comprehensive. In order to achieve this goal, a customer oriented primary nursing procedure process description was developed to be used as a guideline for the primary nurses of the service house in question. This thesis was produced as action research that included three phases.
During the first phase of the action research an electronic survey of primary nursing was made at the working unit of the commissioner. Surveys aim was to study which were the main areas of development in the 24-hour service house. The questions of the survey came from the theoretic framework of the thesis. The survey was sent to 16 primary nurses 11 of them answered it.
The second phase of the action research sought to obtain information on what customer oriented primary procedures include. Answer to this question was searched during a development afternoon that was arranged by the commissioner. In total 13 primary nurses took part in this afternoon event, which was arranged using Learning Cafe method. Themes for the Learning Cafe groups were brought up from the thesis using categorizing.
With the theoretic framework of the thesis, the electronic survey and the development afternoon, a model of customer oriented primary nursing procedures was made as a process description JHS 152. The process description shows the interdependencies between the processes, interactions and results of the processes.
Next, the model was presented to employees. The effectiveness and possible developmental needs were researched with a feedback survey. It was sent to 19 primary nurses and 11 answers were received. Results of the feedback survey showed that the model of customeroriented procedures for the primary nurses was successful. Majority of the survey respondents found that the model was explicit, useful and customer oriented. Majority of the survey respondents found that the procedure answered to the known developmental needs. All of the survey respondents found that the model helps to understand the tasks of a primary nurse. The feedback survey established that the procedure was functional, and no alternations were made.
