Tiivistelmä
Kantaluun apofysiitti eli kansankielisemmin Severin tauti on kasvuikäisten urheilijoiden yleisin kantapääkivun aiheuttaja. Kipu on pahimmillaan rasituksessa ja helpottuu levossa. Vaivan taustalla on apofyysiin eli kasvulevyyn kohdistuva toistuva mikrotrauma, jonka syntyy akillesjänteen vetäessä tätä rasituksessa. Vaivaa esiintyy erityisesti runsaasti juoksuja ja hyppyjä sisältävien lajien harrastajilla.
Kantaluun apofysiitti on viime kädessä itsestään parantuva vaiva, jonka hoitolinja on konservatiivinen. Terapeuttinen harjoittelu aloitetaan jo akuuttivaiheessa isometrisillä harjoitteilla, siirtyen kuntoutumisen edetessä eksentrisiin, konsentrisiin ja plyometrisiin harjoitteisiin. Kuntoutuksen myöhemmissä vaiheissa harjoitteiden tulisi olla yhä lajinomaisempia ja progressiivisesti haastavampia.
Kehittämistyön toimeksiantajana toimii FC Inter, jonka tarpeesta myös itse aihe valikoitui. Opas on suunnattu seuran junioripelaajille ja heidän valmentajilleen, tavoitteena vähentää kantapääkivusta johtuvia pelitaukoja ja nopeuttaa vaivasta kärsivän pelaajan paluuta täysipainoisen lajiharjoittelun pariin.
Kehittämistyön teoreettinen viitekehys perustuu tutkittuun tietoon, jonka pohjalta on luotu videomuotoinen opas kantaluun apofysiitin hoitoon ja kuntoutukseen. Viitekehys etenee pohkeen, nilkan ja jalkaterän anatomisista rakenteista, vaivan etiologiaan ja terapeuttiseen harjoitteluun, sekä harjoitteissa käytettyihin lihastyömuotoihin.
Videomuotoinen opas koostuu alun suoritusohje- ja yleistietodioista, sekä varsinaisista kuntoutusharjoitteista. Tarkoituksena oli luoda helposti saatavilla oleva opas, joka olisi katsojalleen mahdollisimman selkeä ja matalalla kynnyksellä toteutettava. Oppaan harjoitteita pystyy tehdä niin kotona kuin lajiharjoitusten yhteydessä. Calcaneal apophysitis, a condition also known as Sever’s disease, is the most common reason behind heel pain in active children. The pain is worse under stress and relieves at rest. The problem is caused by a repeated microtrauma to the apophysis which is caused by the pulling of the Achilles tendon under stress. The condition occurs the most with young athletes whose sport includes a lot of running and jumping.
The calcaneal apophysitis is ultimately a self-limiting condition and it is treated with a conservative treatment line. The therapeutic exercise training is started in acute phase with isometric exercises following with eccentric, concentric and plyometric exercises in the later stages of rehabilitation. In the later stages of rehabilitation the exercises should be more sport-specific and progressively more challenging.
This thesis has been implemented as development work for the FC Inter ry, from who the need for this kind of rehabilitation video guide came from. The guide has been created for the club’s junior players and their coaches, aiming to reduce breaks from playing and practicing due to heel pain and also to accelerate the young athlete’s return to full sport participation.
The theoretical part of the development work is based on the researched data, on the basis of which also the video guide for the treatment and rehabilitation for Calcaneal apophysitis has been created. The theoretical part proceeds from the anatomy of the calf, ankle and foot to the etiology and rehabilitation of the condition and the forms of muscle work used in training.
The video guide consists of general information of the disease, written instructions and the actual rehabilitation exercises. The goal was to create a guide that is practical and as accessible to people as possible. Exercises can be performed both at home or during sports specific training.
