Tiivistelmä
Euroopan elvytysneuvoston (ERC) voimassa olevat elvytyssuositukset painottavat varhaisen defibrillaation sekä laadukkaan ja keskeytyksettömän paineluelvytyksen merkitystä sydänpysähdyspotilaan ennusteen parantamisessa. Sydänpysähdyspotilaan ennusteeseen vaikuttaa koko hoitoketjun optimaalinen toiminta.
Itä-Uudenmaan pelastuslaitoksella siirryttiin käyttämään LP (LifePak®) 15- defibrillaattorin manuaalitilaa sydänpysähdyspotilaiden kohdalla maaliskuussa 2019. Tätä edelsi pitkä moduulikoulutuspaketti sekä useita elvytyssimulaatioharjoitteita ensihoitajille. Defibrillaattorin manuaalitilan käyttämisen ja elvytysprotokollan muutoksen tavoitteena on parantaa elvytyspotilaiden selviytymistä sekä toteuttaa laadukkaammin elvytyssuositusten mukainen näyttöön perustuva toiminta sydänpysähdyspotilaiden kohdalla Porvoon sairaanhoitoalueen ensihoidossa.
Tämän opinnäytetyön tarkoituksena on kuvata implementoituun uuteen elvytysprotokollaan ja laadukkaaseen paineluelvytykseen liittyvän osaamisen säilyminen vuoden seurannassa Itä-Uudenmaan pelastuslaitoksen ensihoidossa. Tutkimusmetodina käytettiin systemaattista havainnointia. Havainnoinnin kohteena oli tammikuussa 2019 (n = 73) ja 2020 (n = 53) toteutetut identtiset elvytyksen simulaatioskenaariot, jotka toteutettiin ensihoitajille työpareittain. Havainnoinnin apuvälineenä toimi tarkoitukseen luotu tarkistuslista sekä AmbuMan®- simulaationuken antama palaute paineluelvytyksen laadusta.
Tulosten ja vertailun perusteella voitiin tehdä havainto, että elvytysprotokollaan liittyvä osaaminen heikkeni protokollan mukaisessa elottomuuden tunnistamisessa (look-listen-feel-tekniikka), muistinvaraisissa vaiheissa (kello, metronomi, painelusykkeen tunnustelu) sekä turvalliseen defibrillaatioon liittyvässä strukturoidussa kommunikaatiossa. Painelun laatu parani ja painelutaukojen kesto lyheni vertailujakson aikana.
Tulosten avulla Itä-Uudenmaan pelastuslaitoksen elvytyskouluttajat pystyvät paremmin kohdentamaan elvytyskoulutuksen painopisteitä tulevaisuudessa. Osaamisen varmentamista ja seurantaa on tarkoitus jatkaa vuosittain. Current resuscitation guidelines of European Resuscitation Council (ERC) emphasize the importance of early defibrillation and high-quality and uninterrupted CPR in improving the prognosis of a cardiac arrest patient. The prognosis of a cardiac arrest patient is affected by the optimal functioning of the entire treatment chain.
Eastern Uusimaa Rescue Department changed the protocol for use of the manual mode of the LP (LifePak®) 15 defibrillator for cardiac arrest patients in March 2019. This was preceded by an extended modular training package and several resuscitation simulation exercises for paramedics. The aim of using the manual mode of the defibrillator and changing the resuscitation protocol is to improve the survival of patients with cardiac arrest and to implement higher-quality evidence-based action in emergency care in Porvoo Hospital area.
The purpose of this thesis is to describe the retention of competence, related to the implemented new resuscitation protocol and high-quality cardiopulmonary resuscitation in the follow-up year in the emergency care of Eastern Uusimaa Rescue Department. Systematic observation was used as the research method. The object of the observation was identical resuscitation simulation scenarios which were executed in January 2019 (n = 73) and 2020 (n = 53). Paramedics were executing the scenarios as a work pair. The observation tool was a checklist created for the purpose, as well as feedback from the AmbuMan® simulation manikin on the quality of cardiopulmonary resuscitation.
Based on the results and comparison, it could be concluded that there was a slight decrease in resuscitation protocol expertise in terms of protocol-based inexperience detection (look-listen-feel technique), memory phases (clock, metronome, heart rate sensing), and structured communication related to safe defibrillation. On the other hand, in terms of chest compression quality and interruptions, the results improved during the comparison period.
With the help of the results, the resuscitation trainers of Eastern Uusimaa Rescue Department will be able to target the priorities of resuscitation training in the future. The certification and monitoring of competence are planned to continue annually.
