Tiivistelmä
Eturauhassyöpä on miesten yleisin syöpä Suomessa. Ennen diagnoosia potilaalta otetaan paksuneulanäytteet eturauhasesta. Eturauhasen paksuneulanäytteiden käsittely luo haasteita patologian laboratorion histologiseen näytekulkuun. Paksuneulanäytteet ovat ohuita ja hauraita ja niitä on vaikea pitää suorassa näytekasetissa. Ennen kudoskuljetusta paksuneulanäytteet voidaan asettaa esim. filtteripaperille, jotta ne pysyvät mahdollisimman suorassa kuljetuksen ajan. Parafiiniin valussa tulee kiinnittää huomiota näytteen orientaatioon, jotta koko näyte on samalla tasolla. Tällöin myös leikkaaminen helpottuu.
Tämän opinnäytetyön tarkoituksena oli kerätä eturauhasen paksuneulanäytteiden orientaatiolevyn käyttökokemuksia ottamalla Themis Biopsy Chip –orientaatiolevy kokeiluun Varsinais-Suomen sairaanhoitopiirin Tyksin patologian yksikössä. Tavoitteena oli selvittää millaisia muutoksia Themis Biopsy Chip -orientaatiolevy tuo näytteiden laatuun ja käsittelyyn laboratorion eri työpisteissä. Tämän opinnäytetyön menetelminä käytettiin kyselylomaketta ja haastattelua. Themis Biopsy Chip -orientaatiolevy on Histolabin kehittämä eturauhasen paksuneulanäytteille tarkoitettu kudoksen kaltainen kuusiurainen levy. Sen tarkoituksena on helpottaa ja parantaa eturauhasen paksuneulanäytteiden käsittelyä ja laatua laboratorioprosessin aikana.
Opinnäytetyössä hyödynnettiin tutkimuskysymystä ”Paraneeko prostatanäytteen laatu orientaatiolevyn käyttöönoton vaikutuksesta?”. Patologien mukaan näytteiden laatu ja diagnostiikka paranivat. Jopa 62% näytteistä oli laadultaan erinomaisia. Kaikki näytteet olivat laadultaan hyviä tai erinomaisia. Patologit kokivat orientaatiolevyn hyödyllisenä ja parantavan näytteiden luotettavuutta. Prostate cancer is the most common cancer for men in Finland. Thick needle samples from the prostate will be taken before the diagnosis. The handling of prostate thick needle samples challenges histological sample process in the pathology laboratory. Thick needle samples are thin, brittle and difficult to straighten into the sample cassette. Before automated tissue processor, thick needle samples may be placed on, for example, filter paper to keep them as straight as possible during the tissue processing. In embedding, attention should be paid to the sample orientation so that every sample is at the same level. This also makes cutting easier.
The purpose of this thesis was to collect experiences in using the orientation plate for prostate thick needle samples. The experiences were collected in Turku University Hospital pathology unit, Hospital District of Southwest Finland. The aim of this thesis was to find out the changes the Themis Biopsy Chip orientation plate brings to sample quality and handling in different workstations of the laboratory. A questionnaire and an interview were used as a method for this thesis. The Themis Biopsy Chip is a tissue-like orientation plate with six grooves developed by Histolab for prostate thick needle samples. Its purpose is to facilitate and improve the handling and quality of prostate thick needle samples during the histological laboratory process.
In this thesis we used a research question “Will the quality of the prostate samples get better with the orientation plate?”. According to the pathologists the sample quality and diagnostics were improved. Even 62% of the samples were excellent quality. All the samples were either good or great quality. The pathologists thought the orientation plate was useful and it improved the reliability of the samples.
