Tiivistelmä
Muistisairaiden määrän arvellaan kaksinkertaistuvan Suomessa vuoteen 2030 mennessä. Tämän myötä yhä useampi hoitaja kohtaa työssään muistisairautta ja sen tuomia haasteita. Muistisairauksien aiheuttamat käytösoireet ja kognitiivisten taitojen heikkeneminen tekevät hoitotyöstä usein haastavaa. Käytösoireista huolimatta muistisairaalla ihmisellä on oikeus hyvään hoitoon.
Opinnäytetyön toimeksiantajana toimii Turun yliopistollinen keskussairaala (TYKS), Salon sairaala. Tarkoituksena oli selvittää hoitohenkilökunnan kokemuksia muistisairaan potilaan hoitotyöstä. Tavoitteena on lisätä tietoa ja sitä kautta hoitohenkilökunnan osaamista muistisairaan potilaan hoitotyössä. Opinnäytetyö toteutettiin narratiivisena kirjallisuuskatsauksena n=10.
Opinnäytetyössä käytetyt tutkimukset osoittavat, että hoitajien kokemuksiin muistisairaan potilaan hoitotyössä vaikuttavat työnantaja-, vuorovaikutus- ja häiriökäyttäytymis sidonnaiset tekijät sekä hengelliset ja eettiset tekijät. Tuloksista käy ilmi hoitohenkilökunnan kokemuksen olevan yhteydessä hoitajan henkilökohtaisiin ominaisuuksiin, kuten työkokemukseen, koulutustasoon ja ikään. Työnantajasidonnaisten tekijöiden, kuten lisäkoulutusten, hyvän työilmapiirin ja yleisen työviihtyvyyden todettiin vaikuttavan positiivisesti hoitajan kokemukseen. Vuorovaikutustaitojen todettiin lisäävään työhyvinvointia ja henkilökeskeistä hoitoa. Häiriökäyttäytymisen todettiin vaikuttavan negatiivisesti hoitajan kokemukseen ja potilaan huomioimiseen. Hengellisyyden hoidosta henkilökunta koki omaavansa vähän tietoa. Eettiset ongelmatilanteet koettiin hyvin raskaiksi.
Muistisairauksien kasvaessa räjähdysmäisesti on tärkeää löytää keinot hoitajaystävällisen ja laadukkaan hoidon toteuttamiseen. Laajemmat jatkotutkimukset hoitohenkilökunnan kokemuksista ovat tarpeen, jotta epäkohdat ja niihin johtavat syyt saataisiin selville. The total number of people living with memory disorders in Finland is estimated to double until the year 2030. Due to this, a growing number of nurses face memory disorders and their effects. The behavioral disorders of the patient and the decline in the cognitive skills make the Nursing work very demanding. Despite these challenges, the patient has the right to receive good care.
This project was commissioned by the Turku university hospital (TYKS), hospital of Salo. This study aimed to describe and explore the experiences of nursing staff in the care of memory disorder patient, increase information and trough that increase the know-how of the nursing staff. This study was conducted as a narrative literature review (n=10).
The studies utilized in this project show, that the experiences of the nursing staff in the nursing work were affected by employer-, interaction- and disruptive behavior-bound factors and spiritual and ethical factors. The results reveal that the experience was bound by personal characteristics of the nurse, such as working experience, the level of education and age. Employer bound factors, such as further education, good working environment and overall comfort of the working place were shown to affect the experience positively. Interaction skills were shown to improve worker well-being and person-centered care. The disruptive behaviour of the patient was discovered to have a negative effect on the nurse‘s experience and to the observation of the patient. Nursing staff felt they lacked knowledge in the spiritual care of the patient. Problematic ethical situations the staff experienced very heavily.
As the number of patients with memory disorder increases explosively it is important to find a way to produce nursing staff friendly and high-quality care. Broader and further studies on the experience of the nursing staff are needed so that defects and causes behind them would be uncovered.
