Tiivistelmä
Opinnäytetyön tarkoituksena oli kartoittaa teemahaastattelun avulla sairaanhoitajien (n=3) kokemuksia heidän roolistaan lääkärinkierroilla. Tavoitteena oli luoda haastattelujen tulosten perusteella vinkkilista sairaanhoitajille potilaslähtöisyyden edistämiseksi lääkärinkierroilla. Aineisto analysoitiin deduktiivisen sisällön analyysin perusteella. Tutkimuksen toimeksiantajana toimi Varsinais-suomen sairaanhoitopiiri.
Kirjallisuuskatsauksen perusteella lääkärinkierroilla sairaanhoitajalla on kolme erilaista roolia; tietojen esittäjä, potilaan edun valvoja sekä tietojen kirjaaja. Haastatteluiden tulokset peilasivat kirjallisuuskatsauksessa löydettyjä rooleja. Sairaanhoitajalla on tärkeä rooli lääkärinkierroilla. Sairaanhoitaja toimii mm. potilaan edun valvojana, varsinkin tilanteissa, joissa potilas ei itse kykene osallistumaan kiertotilanteeseen. Potilaan edun valvonta koettiin tärkeimmäksi sairaanhoitajan rooliksi. Myös rooli tietojen esittäjänä lääkärinkierrolla koettiin tärkeäksi. Sairaanhoitaja yleensä tuntee potilaat erityisen hyvin, koska he osallistuvat potilaan päivittäiseen hoitoon. Roolit edellyttävät hoitajalta ammattitaitoa, asiantuntemusta sekä potilaan tilanteen tuntemista. Tietojen kirjaamisen rooli koettiin jaetuksi lääkäreiden kanssa. Vaikka roolit tunnistettiin haastatteluissa, jäi niiden määrittäminen kuitenkin vaillinaiseksi.
Tuloksista ilmeni, että sairaanhoitajan roolit, sekä niihin liittyvät toimet ovat vahvasti sidoksissa potilaslähtöisyyteen. Sairaanhoitaja voi vaikuttaa positiivisesti potilaan kokemuksiin lääkärinkierrosta ja sitä kautta edistää potilaslähtöisyyttä. Tärkeimpinä keinoina potilaslähtöisyyden lisäämiseksi mainittiin potilaan kunnollinen huomioiminen, valmistaminen kiertoa varten, sekä osallistuttaminen lääkärinkiertoon. Haastattelun tulosten perusteella on tehty kymmenen kohdan vinkkilista potilaslähtöisyyden edistämiseksi.
Jatkossa tulisi selvittää rooleille tarkemmat määritelmät, sekä kartoittaa ovatko sairaanhoitajat tietoisia rooleista, sekä niihin liittyvistä tehtävistä. Myös olisi hyvä selvittää onko sairaanhoitajilla yhtenäinen käsitys siitä, mitä potilaslähtöisyys tarkoittaa. The purpose of the study was to research how nurses experience their role as a part of ward rounds by using theme interviews as method (n=3). The aim was to use the interview results to create a list of tips for nurses for promoting patient orientation during the ward rounds. The results were analyzed with deductive content analysis. The study was commissioned by Hospital District of Southwest Finland.
Based on a literature review nurses have three roles in a ward round: presenting information, protecting the patients’ interests and recording information. The interviews reflected these roles. The nurses have an important role in the ward rounds. They operate as protectors of the patients’ interests, especially when the patients themselves are not able to participate in the situation. The interviewed nurses found the role as protector of patients’ interests to be the most important one. In addition, their role in presenting information during the ward round was seen as important. Of the health care staff, the nurses are generally most familiar with the patient’s status as they care for the patient on day-by-day basis. These two roles require the nurses to have expertise, professionalism and knowledge of the patient’s current state. The third role as information recorder was considered to be shared with the doctors. While the roles were recognized in the interviews, their definitions remained somewhat unclear.
The results also indicated that the roles the nurses have, as well as the actions related to them, were closely tied to patient orientation and patient oriented care. The nurses can positively affect the patients’ experiences during the ward rounds and make them more patient oriented through their work. Properly taking the patient into account, preparing them for the round and engaging them during the ward round were considered to be the most important means to increase patient orientation. Based on the results from the interview a list of 10 tips was formed to improve patient orientation.
Future research should better define and clarify the different roles nurses have during ward rounds and map out if nurses are aware of the roles and different tasks related to them. In addition, it should be studied if nurses’ understanding of patient orientation is uniform.
