Tiivistelmä
Enterokokit ovat yleisiä sairaalaperäisten virtsatieinfektioiden aiheuttajia. Niistä yli 95 %:ssa aiheuttajana ovat pääasiassa Enterococcus faecalis- ja Enterococcus faecium 'lajit, joista arabinoosipositiivinen Enterococcus faecium on resistentimpi. Keski-Suomen keskussairaalan kliinisen mikrobiologian laboratorio vastaa kaikki arabinoosipositiiviset enterokokit lajinimellä Enterococcus faecium, koska laboratoriossa ei ole käytössä tarkempaan lajitunnistukseen kykenevää diagnostista testimenetelmää. Käyttökelpoisen testimenetelmän puuttumisen vuoksi ryhdyttiin tutkimaan neljän eri kaupallisen testikitin toimivuutta ja luotettavuutta enterokokkilajien tunnistuksessa sekä millä edellytyksillä jokin näistä tutkituista testimenetelmistä soveltuisi diagnostiseen analytiikkaan. Käytetyt testimenetelmät olivat Api 20 Strep, Rapid ID32 STREP, RapIDTMSTR System ja DiatabsTM 'testikokonaisuus. Näytemääränä tutkimuksessa oli 46 virtsa- ja veriviljelynäytteistä eristettyä, Enterococcus faecium'lajiksi vastattua bakteerikantaa. Näistä kolme olivat tunnettuja kontrollikantoja, Enterococcus faecium, Enterococcus faecalis ja Enterococcus gallinarum. Varmoiksi Enterococcus faecium 'lajitulkinnoiksi luokiteltiin sellaiset tapaukset, joissa Api 20 Strep 'testin tunnistusprosentti oli vähintään 96 ja kun Diatabs -testikokonaisuudesta tiettyjen osioiden tulokset tukivat tätä. Tutkimuksen perusteella voidaan tehdä se johtopäätös, että Api 20 Strep 'testi sekä Diatabs 'testikokonaisuudesta tietyt osiot tarjoavat kätevän ja objektiivisen tavan tunnistaaEnterococcus faecium, Enterococcus faecalis ja jokin muu nimeämätön enterokokkilaji. Mikään tutkimuksessa käytetyistä testimenetelmistä ei pystynyt tutkimuksenperusteella luotettavasti tunnistamaan harvinaisempia enterokokkilajeja, kuten Enterococcus gallinarum, Enterococcus casseliflavus tai Enterococcus avium. Enterococci bacteria is the most common cause of hospital based urinary infections. Over 95% of these have been found to have caused mainly by Enterococcus faecalis and Enterococcus faecium species, of which arabinose positive Enterococcus faecium is the most resistant. The Clinical Microbiology laboratory of the Central Hospital of Central Finland labels all arabinose positive enterococci bacteria by the name Enterococcus faecium, because the laboratorydoes not have in its use a more accurate species identification test facility. Due to the lack of usable testing facility the testing of four separated commercial test kits was commenced for the usefulness and reliability for the identification of the enterococci and what proof was needed to decide which testing approach would fit the diagnostic analysis. The methods tested were Api 20 Strep, Rapid ID 32 Strep, RapidTM STR system and DiatabsTM test field. The strains tested included 46 urine and blood samples selected from the identified Enterococcus faecium strains. Out of these three were from the recognised control group, Enterococcus faecium, Enterococcus faecalis and Enterococcus gallinarum. The incidentswhich were then interpreted according to their strand as being 'certain' Enterococcus faecium were the cases, in which Api 20 Strep id% was at lest 96% and where some Diatabs test parts confirmed these results. Based on the results of the testing it can be concluded that Api 20 Strep -test and also some of the Diatabs test parts offers useful and objective method to identify Enterococcus faecium, Enterococcus faecalis and some other unnamed species of enterococci. None of the test methods used in the research were able to reliably recognise more unusual enterococci species like Enterococcus gallinarum, Enterococcus casseliflavus or Enterococcus avium.
