Tiivistelmä
Tässä kehittämistyössä tehtiin opetusvideon Savonia-ammattikorkeakoulun opiskelijoille sekä ensihoidossa työskenteleville ensihoitajille, pelastajille sekä sopimuspalokuntalaisille. Kehittämistyön tavoite on helpottaa ensihoitajien päätöksentekoa potilaan tukemisen tarpeesta traumapotilaan ensihoidossa, sekä parantaa potilaan hoitoa lisäämällä ensihoitajien osaamista. Toimeksiantajana kehittämistyössä oli Savonia-ammattikorkeakoulu. Tuotosta saa käyttää myös muut oppilaitokset sekä pelastuslaitokset. Video ladataan myös YouTube videotoistopalveluun kaikkien katsottavaksi.
Vammapotilaan hoito on oleellinen osa ensihoitotyötä. Vammapotilaan hoidossa on suuria haasteita yksiselitteisiä oikeita vastauksia ei ole, eikä tilanteisiin ole valmista protokollaa. Vammapotilaan tukeminen on iso osa vammapotilaan hoitoa, mutta väärin perustein tuettu potilas voi huonontaa potilaan ennustetta ja näin olla jopa hengenvaarallinen potilaalle. NEXUS-kriteerit auttavat tässä vaativassa ja stressaavassa tilanteessa olevia hoitajia tekemään päätöksen potilaan kaularangan tukemisen tarpeesta, kun muistaa videolla esitetyt viisi NEXUS-kriteeriä. Näitä kriteereitä noudattamalla potilaan kaularangan voi tukea tai jättää tukematta näyttöön perustuvaan tutkimukseen luottaen. Kehittämistyö rajattiin potilaan kaularangan tukemiseen. Kehittämistyössä ei otettu kantaa tukemisvälineiden oikeasta käyttötavasta, jolla voi myös olla potilaalle merkittävää haittaa.
NEXUS-kriteerit ja toimintaohjeet perustuvat sen aikaisiin hoitosuosituksiin ja ohjeisiin. Tutkimusten mukaan oikein käytetyt NEXUS-kriteerit lisäävät potilasturvallisuutta ja luo yhtenäisemmän toimintamallin ensihoitoon. Kehittämistyön tarkoituksena oli tehdä tuotos ensihoidon opiskelijoille, ensihoitajille, pelastajille sekä sopimuspalokuntalaisille. Videolla havainnollistetaan toimintamalli, jota on helppo toteuttaa kohdattaessa vammapotilas. Kehittämistyön tavoitteena on lisätä ja yhtenäistää tietoa vammapotilaan hoidosta. Näin toiminta perustuu tutkittuun tietoon ja potilasturvallisuus lisääntyy.
Video tehtiin mahdollisimman yksiselitteiseksi ja helposti ymmärrettäväksi, jotta kynnys käyttää kriteerejä ensihoidossa, olisi mahdollisimman alhainen. This thesis is a functional thesis, the output of which was a video for Savonia University of Applied Sciences. The video was targeted at Savonia University of Applied Science students, paramedics and firefighters. The goal of the thesis is to facilitate the decision making about the need for supporting the trauma patient´s cervical vertebrae and thus to improve the patient care by improving paramedics´ expertise as a whole. The video was made for Savonia University of Applied Sciences but it can be used by other educational organisations and emergency services. The video was also uploaded to YouTube for everyone.
Trauma patients´ treatment is an essential part of paramedics´ work. There are a lot of challenges with trauma patient treatment because of the lack of easy answers and there are no ready protocols for the situations. Supporting patients' cervical vertebrae is a big part of trauma patients' treatment but wrong decisions can impair the prognosis and therefore be lethal for the patient. Nexus criteria help with decision making in this difficult and stressful situation when you remember the five NEXUS criteria on the video. Using these evidence-based criteria you can choose or choose not to support the cervical vertebra. The thesis deals with supporting the cervical vertebrae but it does not deal with the right usage of supporting tools which can also be harmful for patients.
The NEXUS criteria and procedure are based on the general evidence-based guidelines and instructions at this time. Researches show that the right usage of NEXUS criteria increases patient safety and creates a consistent operating model in the field of prehospital care. The goal of the thesis was to make a video targeted at Savonia University of Applied Science students, paramedics and firefighters. The video demonstrates an operating model that can be easily used when treating a trauma patient. The goal of the thesis was to increase and unite knowledge of trauma patient treatment. It is evidence-based and increases the safety of the patient.
The video was made as easy and simple to understand as possible so that the criteria shown on the video would be more easily used.
