Tiivistelmä
Tässä opinnäytetyössä toteutetun kehittämistyön tarkoituksena oli kehittää ja selkiyttää tehostetun avohoidon prosessikuvausta psykiatrian klinikan vaativan hoidon ja kuntoutuksen osastolla. Tehostetussa avohoidossa sovelletaan ACT-mallia, avohoitoon kuuluu palveluohjausta ja yhtenä hoitomenetelmänä käytetään psykoedukaatiota. Kehittämistyön tavoitteena oli parantaa tehostetun avohoidon laatua luomalla yhteinen toimintatapa tehostettuun avohoitoon ja laatia tehostettuun avohoitoon selkeä asiakkaan hoitopolku.
Kehittämistyö toteutettiin vaativan hoidon ja kuntoutuksen osaston henkilökunnan, psykiatrian klinikan erityistyöntekijöiden ja tehostetun avohoidon yhteistyökumppaneiden kanssa. Kehittämismenetelmänä oli toimintatutkimus. Kehittämistyö tehtiin osallistavalla otteella, johon toimintatutkimus sopii hyvin. Ensimmäiseksi tehostetun avohoidon vastaavat hoitajat pohtivat aivoriihessä ongelmat valmiiksi työpajoihin. Aineiston keruu menetelmänä käytettiin learning cafe -menetelmää. Learning cafe-työpajoja järjestettiin kaksi kappaletta saman sisältöisesti. Aineisto kerättiin learning cafe työpajoissa pöytäliinoihin ja se analysoitiin sisällön analyysillä.
Tuloksissa esitellään tehostetun avohoidon perusosat. Tehostetun avohoidon perusosina olivat tehostetun avohoidon toiminta-ajatus, hoitajan tehtävän kuva, asiakkuuden määrittely ja tehostetun avohoidon hoitotoimenpiteet. Perus-osien pohjalta luotiin tehostetun avohoidon prosessikuvaus. Prosessikuvauksessa ilmenee asiakkaan hoitopolku tehostetussa avohoidossa. Jatkokehittämishaasteena on tehostetun avohoidon kehittäminen vastamaan suurempaa asiakasjoukkoa. Tämä kehittämistyö antaa tähän hyvän pohjan.

The study can be described as a development research. The ACT model is used in enhanced out-patient care including case management and psychoeducation. The aim of this research was to improve the quality of intensified outpatient care by creating a common approach to intensive outpatient care and by establishing a clear patient care path for enhanced outpatient care.
This research was carried out among the staff of the demanding care and re-habilitation department, the special needs staff of the psychiatric clinic, and the partners in the enhanced outpatient care. The method used was action re-search. The research was conducted by using participatory methods, suitable for an action research. A group of enhanced outpatient responsible caregivers met for brainstorming. During the brainstorm sessions the problems were identified and they were discussed in a workshop. In the workshop the Learning Café method was used. Two similar learning cafés were organized for the workshops to have more participants. The notes from each table in the Learning Café were collected and analyzed by content analysis.
The results revealed basic elements of enhanced outpatient care. The basic elements are the operational idea of intensified outpatient care, description of the caregiver's task, the definition of the patient, and the care measures for enhanced outpatient care. Based on the basic elements, a process description of enhanced outpatient care was created. The process description shows the patient’s care path in enhanced outpatient care. A potential further development challenge would be to develop enhanced outpatient care to reach a larger number of patients. This development research may provide good foundations for that.
